Job Description

Mainly work in the patient home. Work hours: day, overnights or weekends

Clinical Responsibilities

  • 8-12 hour shifts – Day or night shifts.
  • Provide extended hours of skilled nursing care in a patient’s home.
  • Provide tube feeding, tracheostomy, suctioning, oxygen support through ventilator
  • Provide care to patients who require drug therapy
  • Work with other field staff in the home to assess the condition of the patient (if RN) and develop an individual plan of care.
  • Implement a patient’s plan of care and evaluate the plan’s effectiveness.
  • Educate patient and family members on the patient’s condition and plan of care.
  • Provide medication management.
  • Monitor patient’s condition and response to treatments, recording observations and communicating with the healthcare team.
  • Communicate with the patient, medical practitioners and other appropriate home health field staff to plan, implement and assess a holistic intervention program.
  • Direct and coordinate all nursing care for patients based on established clinical nursing practice standards.
  • Comply with professional, ethical and legal standards.

Job Qualifications

  • Licensure: Minnesota RN or LPN License – Required

  • Experience: required: at least 1 year of nursing experience preferred

  • Good problem solver to make independent decisions

  • Effective communicator with staff, client families, and outside agencies

  • Ability to prioritize duties and work independently

  • Clinical competence to utilize nursing process to perform physical and cognitive assessments
